Oregon Provisional Instruction Permit Requirements
- Study the Oregon Driver Manual
- Fill out the application (with parent's signature)
- Take it to a DMV office
- Show proof of your physical address and legal presence/identity
- Pass a vision test
- Prepare for the knowledge test
- Pass the knowledge test
- Pay the fees and get your photo taken
Oregon Provisional Driver's License Requirements
- Be 16 or 17 years old
- Prepare for the knowledge test
- Have had an instruction permit for at least six months
- Get driver training (optional)
- Complete 100 hours of supervised driving or 50 hours if you have taken and passed an approved driver education program
- Schedule a test with DMV by calling 503-945-5000
- Fill out the application (with parent signature)
- On testing day, take it to the DMV office where you scheduled your test
- Show proof of your physical address and legal presence/identity
- Pass the knowledge test

- Pass the drive test
- Pay the fees and get your photo taken
